Consent and Liability Agreement

By participating in any Shoot & Shape Athletics training sessions, I acknowledge and agree to the following:

• I understand that basketball and physical training involve inherent risks, and I assume all responsibility for any injuries or accidents that may occur during the training.

• I confirm that Shoot & Shape Athletics, its coaches, and staff are not liable for any injuries, accidents, or health-related issues that may arise from my participation in these activities.

• I certify that I have no serious medical conditions or health concerns that would prevent me from safely participating in physical exercise and basketball training.

• I agree to provide at least a 24-hour notice for any cancellations. Failure to do so may result in fees or charges as per Shoot & Shape Athletics' policies.

By signing up, I accept these terms and participate at my own risk.
